That’s what we believe at Care Alliance Health Center. As the leading provider of medical and dental services to those experiencing homelessness or living in and around public housing in Cleveland, Ohio, we understand the impact that health care can have on a person’s life. We work hard every day to make this impact a reality for those who need it most.

This vision was affirmed through our recent recognition by the National Committee for Quality Assurance as a Level Three Patient-Centered Medical Home (PCMH), the highest level of recognition. PCMH is an innovative model for improving primary care by organizing care around patients, working in teams of medical providers and support staff, coordinating and tracking care over time, and utilizing technology to share information amongst providers and with patients.
