Diabetes Program
According to the American Diabetes Association, there are 20.8 million children and adults in the US with diabetes -- and nearly one-third of them (or 6.2 million people) do not know it! The disease is most common amongst African Americans. |

Care Alliance is a part of the National Health Disparities Collaborative whose goal is to prevent health disparities in underserved areas. Through the Care Alliance Comprehensive Diabetes Program, individuals diagnosed with diabetes receive comprehensive medical treatment and free diabetic supplies such as a glucometer, lancets, alcohol pads, testing strips, and medications. They also receive one-on-one educational sessions and continuous follow up to help the patient manage their diabetes.
The first Friday of every month, Care Alliance hosts a Diabetes Educational Program at noon at the Health Center at 1530 St. Clair Avenue. Here, patients are invited to learn about diabetes and are encouraged to participate in activities from exercise instruction to nutritional counseling facilitated by the Diabetic Association of Greater Cleveland. Special screenings are scheduled for podiatry care and eye care.
