Outreach Services
Providing care through outreach is an important element to serving the homeless population. Many of the homeless patients we serve are trauma survivors and use isolation as a primary defense mechanism and coping skill to maintain safety. Their tendency to avoid any situations in which they would feel vulnerable creates significant impediments to requesting treatment and engaging in care. To address this, Care Alliance has an outreach team that works to build trust with individuals experiencing homelessness in environments where they are most comfortable (locations where they sleep or obtain services) and eventually bridge them into care.
Care Alliance works closely with safety-net agencies throughout Cleveland to provide clinics in locations where the homeless population is accessing services. Currently, Care Alliance offers clinics at various locations, including:
- City Mission
- Community Assessment and Treatment Services
- Lutheran Metropolitan Ministry, 2100 Lakeside Avenue Men’s Shelter
- Norma Herr Women’s Center
- Salvation Army Adult Rehabilitation Center
- St. Malachi Center
- West Side Catholic Center
