Care Alliance is committed to providing comprehensive services that address the health of the whole person.  We understand that our patients often face multiple barriers to care including a lack or absence of housing, food, transportation, health insurance, and employment. 
As a result, in addition to our medical and dental care, we provide case management and behavioral health services to address the unique needs of our patients.

Our case management team assists clients to attain basic necessities such as shelter, food, and clothing, and reducing the barriers to care by supporting them in locating eligible housing opportunities, obtaining identification, transportation, location of employment opportunities, and/or training to gain employment skills.  Our chemical dependency team provides group sessions for appropriate clients, while providing one-on-one counseling to referred clients; while our mental health team performs individual assessments with clients and also provides additional referrals if necessary.
