Care Alliance established its Street Outreach Program in order to carry out our mission more fully by providing health care to individuals experiencing homelessness who are living in shelters, on the streets, in homeless camps or abandoned buildings who are either uncomfortable with the traditional healthcare setting, or face significant barriers to accessing care.
Care Alliance is the only health care provider in the City of Cleveland that focuses on the unique health care needs of individuals and families who are experiencing homelessness. As such, we have services designed specifically for reaching out to this transient population; linking them with necessary health care services, obtaining personal identification, public assistance and stable housing for them, and bringing them in to our main clinics where they can access the full scope of services needed.
In attempt to reach a greater number of homeless persons who are not connected with safety net services, the Outreach Team has collaborates with agencies throughout the citywhere homeless individuals tend to congregate so that they may provide medical and supportive care. These outreach sites have medical clinics and include:
Lutheran Metropolitan Ministries' Men’s Shelter at 2100 Lakeside;
The Salvation Army ARC Shelter;
City Mission;
CATS (Community Assessment and Treatment Services, Inc.);
West Side Catholic Center;
St. Malachi; and
Catholic Worker Storefront on Lorain Avenue.
