Primary Medical Care for All

At Care Alliance Health Center (CAHC), our goal is to improve the quality of life of our patients by providing the comprehensive medical care needed to achieve and maintain health. Primary medical care is provided by a multi-disciplinary team of providers, nurses, and support staff who use a lifespan approach to care. Our dedicated team ensures that services are provided in a dignified and culturally sensitive manner.

In 2013, CAHC was recognized by the National Committee for Quality Assurance as a Level Three Patient-Centered Medical Home (PCMH), the highest level of accreditation. PCMH is an innovative model for improving primary care by organizing care around patients, working in teams of medical providers and support staff, coordinating and tracking care over time, and utilizing technology to share information amongst providers and with patients. Our diverse team of staff work together to ensure holistic care to each and every one of our patients.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n), and a deemed FTCA facility.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain Health or Health-related claims, including medical malpractice claims, for itself and it’s covered individuals.
