Our primary care team supports patients in managing chronic illness. Chronic illness prevalence is on the rise – no matter your income level. But at Care Alliance, because of the unique lifestyles that our patients acquire due to living in extreme poverty, they are even sicker than the general population. In 2010, nearly fifty percent of our medical patients had a chronic illness as their primary diagnosis. The most common chronic illness is hypertension, followed by diabetes. Treating patients with a chronic illness is very expensive because our chronically ill patients must return regularly for care and often need an expensive medication regimen. As a result, we often look to the Cleveland community for help supporting our programming for this group.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n), and a deemed FTCA facility.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain Health or Health-related claims, including medical malpractice claims, for itself and it’s covered individuals.
