School-Based Health

The Care Alliance School-Based Health program is designed to improve the health of children and youth by improving access to health care by bringing health care to the students within the Cleveland Metropolitan School District (CMSD). Research has shown that when health and education come together, great things happen. Attendance improves. Conditions like asthma, obesity or diabetes are better managed. Behavioral health issues get quick, expert attention. Our vision is that all children are healthy and achieving their fullest potential.

Services offered:
-Medical assessments
-Well-child visits
-Routine office visits
-Sick visits
-Sports and work physicals
-Basic lab testing
-Health education
-Behavioral Health Assessments
-Confidential health sex visit for teens (pregnancy testing & STI testing)
-Link student and families to ongoing and primary preventive Care

-Open to all students at participating CMSD schools
-Parent/guardian must complete and sign consent form
-Consent forms active while student is enrolled in CMSD
-CMSD School Nurse coordinates visits
